The Gemara in Sanhedrin says that Bnai Keturah are obligated in Mila. Rashi says that it's only referring to the actual sons of Avraham and not for future generations. However, the Rambam says that it applies for all the generations. He concludes that now the Bnai Ketura are mixed up with the Bnai Yishmael, therefore they're all obligated to have a Mila because of the Safeik, but they don't get killed if they refuse, since it's only a Safeik.
 
The Shagas Arye asks, why are they obligated since the majority are exempt? The son of the Noda B'Yehuda assumes the question is that since most are Yishmaelim, we should say that each individual can say that he's from the majority and is exempt? To this he says that the Halacha that we give the status of the majority is a Chidush (since they could be likely from the minority too) and thus only given to Jews. But by Goyim, there is no exemption to assume that they're from the majority, therefore everyone is obligated
 
The Minchas Chinuch says that the Nodah B'Yehuda's son learnt the question wrong. The question is, since Sancherev had mixed up the entire world, and therefore we don't know which people were the descendants of any nation. Therefore if you want to say there is no Halacha of Rov (majority) by Goyim, then they all need to have Mila
